Why starting HRT at 52 vs. 62 changes your heart disease risk: the 10-year window explained

Hormone therapy's effect on your heart depends on when you start it relative to menopause. Complicated plaques with inflammatory cores and thin fibrous caps can rupture when estrogen affects clotting factors and inflammatory signaling in ways that don't matter in healthy vessels but become dangerous in diseased ones.

It's not just about having some plaques versus none. It's about the stage and complexity of those plaques.

What did the clinical trials actually show?

ELITE was the only randomized controlled trial designed specifically to test the timing hypothesis. Women starting estradiol less than six years after menopause showed significantly reduced atherosclerosis progression. Women starting 10 or more years after menopause showed no benefit.

Reanalysis of Women's Health Initiative data stratified by age and time since menopause found that excess coronary heart disease risk with HRT was confined to older women or those more remotely menopausal. Younger women within a decade of menopause showed trends toward cardiovascular benefit.

Hormone therapy started in younger women or soon after menopause reduces death and heart disease risk—not just surrogate markers like plaque thickness, but actual deaths and heart attacks.

If you're beyond the 10-year window or over 60, the cardiovascular risk-benefit shifts. Symptom relief may still justify use—severe hot flashes, sleep disruption, vaginal atrophy, quality of life—but heart protection is no longer part of the equation.
